Open enrollment for 2003 benefits ends Oct. 31

Time is running out to sign up for 2003 benefits, with the open enrollment period ending Thursday, Oct. 31.

People who plan to make changes to their benefit enrollments or to enroll in a health care or Dependent Care Flexible Spending Account for 2003 are encouraged to promptly complete, sign and return the Open Enrollment Personal Enrollment Worksheet.

No adjustment period is scheduled for this year. Employees will have an opportunity to correct errors on their confirmation statements. However, they will not be able to enroll on the confirmation statement in any benefit plan that they did not enroll in using the worksheet.

To be considered timely, the worksheet must be faxed or postmarked by midnight Oct. 31. All pages should be returned with the worksheet. The recommended way to return the form is to fax it to (734) 763-0363. The form also can be sent through Campus Mail or U.S. Mail to HRRIS Benefits Transaction Team, 4005 Wolverine Tower, 3003 South State Street, Ann Arbor, MI 48109-1281. Employees should keep a copy with their personal records, along with copies of other mailings from the Benefits Office. In deciding whether to make any changes to benefits for 2003, employees should review the worksheet to make sure it is accurate. They should check the spelling of names and accuracy of birth dates, and note any changes in employee contributions to the benefit plans for 2003, such as medical and life insurance plans.

If they are happy with the benefits they currently are enrolled in and do not wish to enroll in a Flexible Spending Account for 2003, they do not need to return the enrollment worksheet or take any other action. The current benefit enrollments will continue into 2003, except that prescription drugs will be covered under a new Prescription Drug Plan.

If employees wish to participate in Flexible Spending Accounts, they must re-enroll every year, even if they wish to contribute the same amount of money to the account. They will need to return all pages of the worksheet by midnight Oct. 31. AFSCME members will participate in flexible benefits beginning Jan. 1 and will have more benefit plans from which to choose. All the choices are listed on their worksheets. Participants need to follow the instructions on the worksheet to choose their benefits, and do not have to totally re-enroll in benefits for 2003. Confirmation statements will be mailed to home addresses in late November to all benefits-eligible faculty and staff, whether or not changes were made during open enrollment. Recipients are asked to do the following:

· Carefully review the confirmation statement to make sure it correctly lists the choices they made. Check the spelling of names and accuracy of birth dates. If the statement is correct, they do not need to do anything. Forms should be kept with personal records.

· If the confirmation statement is not accurate, the incorrect information should be circled in black ink, with the correct information printed clearly next to it. The confirmation statement should be returned by Dec. 5. Forms should be faxed to (734) 763-0363 or sent by Campus Mail or U.S. Mail to HRRIS Benefits Transaction Team, 4005 Wolverine Tower, 3003 South State Street, Ann Arbor, MI 48109-1281.
