Today is last day to get $50 for completing health questionnaire

Today is the last day to complete the MHealthy Rewards confidential health questionnaire. By doing so, qualifying benefits-eligible faculty and staff will:

(Note: Both sites will require benefits-eligible employees to sign in using their uniqname and password.)

• Get a personalized health report
• Earn a $50 before-tax incentive
• Get a chance to win a $500 gift card

More than 48 percent, or 18,800 benefits-eligible employees, have already completed the health questionnaire. The universitywide goal is 50 percent.

"The health questionnaire only takes 20 minutes and can be very rewarding," says Janice Gasaway, MHealthy Rewards program lead. "You'll get a report on your current health, plus an extra $50 in your paycheck. And if you completed the health questionnaire in the past, you will see time-over-time results."

The health questionnaire is confidential and individual health information is always private. To protect privacy, U-M contracts with StayWell Health Management, a national provider of health improvement services, to administer the questionnaire.

Once an employee completes the health questionnaire, they also will have the opportunity to earn another $50 by completing a Rewards-eligible healthy activity by July 12. Dozens of activities qualify, including Weight Watchers, nutrition consults, personalized exercise sessions, alcohol management programs and more.
